Job Description
This position is responsible for identifying improvement opportunities across Brown & Toland that support the activities associated Hierarchical Condition Category (HCC) initiatives; utilize various data points and tools, internally and externally which focus on HCC diagnosis capture and documentation to ensure successful integration of HCC initiatives; perform encounter audits and medical record reviews; positively identify and influence practice patterns and solutions.
Skills Required

|
Certified Professional Coder (CCS or CPC through AHIMA/AAPC)
|

|
Minimum of two (2) years coding experience in HCC related activities and general auditing experience.
|

|
Ability to identify elements of a medical record's structure and content; code abstracting and the ability to convert hand written medical charts.
|

|
Extensive knowledge of ICD 9 and CPT classifications and coding of diagnoses and procedures.
|

|
Ability to problem solve and to communicate in a professional manner with physicians, staff and other health care professionals.
|

|
In depth knowledge of medical terminology, human anatomy/physiology, and pharmacology.
|

|
Excellent written and verbal communication skills are essential.
|

|
Ability to multitask and effectively adapt under deadlines.
