Congestive Heart Failure Management
Program Description
More than five million people in the Unites States have heart failure. Fortunately, heart failure is manageable with Brown & Toland's free and comprehensive Congestive Heart Failure (CHF) Management Program, designed to help you stay out of the hospital. The goal of the program is to help you learn to manage your health.

Brown & Toland patients who enroll in this FREE program are paired with a clinical nurse specialist who will help you with the following:

  • Provide tips to help you with medications;
  • Monitor signs and symptoms;
  • Help you discover ways to change your lifestyle;
  • Provide you with guidance on maintaining a heart healthy diet;
  • Coordinate care between multiple healthcare providers;
  • Help you find other community resources; and
  • Assist you with accessing your health plan benefits and services.

As a participant you may receive the following:

  • A published and informative booklet for heart failure patients;
  • A helpful weight and medication chart;
  • A medication pill box; and
  • A free weight scale

Blood Pressure Screenings
More than one in three adults have high blood pressure. This silent disease may result in stroke, heart attack, heart failure and arterial aneurysm. It is also a leading cause of kidney failure. Keep track of your blood pressure at these drop-in screenings. Third Fridays, 1 p.m. to 2 p.m., at 2100 Webster Street, San Francisco, Suite 100. For more information, call 415.923.3155.

Stroke Survivor Support Group

This free support group is designed to aid the recovery of stroke survivors by providing a safe and supportive atmosphere where individuals are able to share their experiences. Those at any stage of recovery are invited to attend. First Thursdays, 1 p.m. to 2:30 p.m. at 2100 Webster Street, San Francisco, Suite 119, Conference Room. Call 415.923.3155 for more information.
