Consumer Website Privacy Policy

Effective Date: July 1, 2004, Revised September 26, 2013

Commitment to Privacy

Brown & Toland Medical Group ("Brown & Toland") respects the right to privacy of all visitors to its website and is committed to maintaining the privacy of those who use our website. This Consumer Website Privacy Policy is designed to assist you in understanding how we collect and use the personal information you provide to us and to assist you in making informed decisions when using our website.  Brown & Toland may modify this Consumer Website Privacy Policy at any time, and such modifications shall be effective immediately upon posting the modified policy on this website. You agree to review this policy periodically to be aware of such modifications, and your accessing or using our website constitutes your acceptance of this policy as it appears at the time of your access or use.

Information We Collect

When you visit our website, you may provide us with two types of information: 1) personal information you knowingly choose to disclose and 2) website use information.

Personal information is any information you've provided us through our website that uniquely identifies you, such as name, social security number, health plan ID number, medical record number, date of birth, telephone number, address, or email address, to name a few.

Website use information is information that is gathered automatically by Brown & Toland to track website activity, including the number of users and the pages visited. Website use information includes internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, and date/time stamp. Website use information does not identify individual users.

How We Use Information

Brown & Toland does not collect personal identifying information about you through our website unless you choose to disclose it.  Brown & Toland limits the sharing and use of personal information that you submit to Brown & Toland personnel who need this information to respond to your question or request, to provide services to you, and to address inappropriate uses of our website.

Brown & Toland uses the personal information that you submit to us via our website to administer our business activities and to respond to your inquiries. We will not sell, lease, rent, or license your personal information to a third party, unless you authorize us to do so, or except as required by law. We may provide your personal information to health care entities involved in providing health care to you, such as your health plan or physician. In such case, Brown & Toland will disclose personal information in compliance with all applicable laws and regulations.

Publications

If you provide an email address to subscribe to a Brown & Toland sponsored publication or newsletter, we may use your email address to send you communications about other Brown & Toland products and services.  Your email address may be provided to Brown & Toland’s contracted email management vendors for purposes of delivering these communications; however, your email address is not shared with businesses outside of Brown & Toland for any marketing purposes.

Surveys

Brown & Toland may occasionally ask site visitors to complete a survey to help us understand the needs of our visitors so that we can improve our site. The data collected from these surveys is aggregated and stripped of identifying information.  The information may be shared with third parties with whom we have a business relationship.  Those third parties have agreed to keep all data from surveys confidential.

IP Addresses

The Web server automatically collects the IP address of the computers that access our site. An IP address is a number that is assigned to your computer when you access the Internet. It is not truly personally identifiable information because many different individuals can access the Internet via the same computer.

Cookies

Brown & Toland collects information about visitors to its site using “cookies” (also known as an HTTP cookie, web cookie, or browser cookie), which are small pieces of data sent from our website and stored in a user’s web browser while a user is browsing a website.  The use of cookies helps us recognize a repeat visitor and offer the visitor a set of services or information requested in a previous visit. We use session cookies to track a visitor's path through our website during a visit, to help us understand how people use our website. Cookies are not used on non-secure portions of Brown & Toland's website.
