The Case Management Department is a team of highly qualified registered nurse (RN) case managers who help patients achieve an optimal level of wellness and function.

Case Management Functions:

• Assessment and coordination of services and resources necessary to meet the patient's overall care requirements.
• Coordination and communication with the health care team, vendors, and family with care planning and identification and implementation of optimal recovery goals.
• Short and/or long term care planning.
• Identification of high quality and cost-effective alternatives of care whenever possible.
• Identify and advise on the optimum use of health care benefits.
• Research and refer patients to sources for alternative funding and community resources
