Transforming the delivery of primary care with a patient-centered philosophy

Inspired by our patients to provide the best care possible, Brown & Toland established My Health Medical Group in 2012 as a patient-centered medical home. The patient-centered medical home concept has become, in recent years, a widely accepted model for how to organize and deliver primary care.

The term “home” is easy to misinterpret; the model provides the patient better access to care as well as a doctor and a team of healthcare professionals who work together to coordinate care. By forging healthcare partnerships between the physician and patient, patient-centered medical homes have shown to improve care coordination and achieve better quality outcomes. 

Patient-centric team approach

At My Health Medical Group (MHMG), located in San Francisco, healthcare teams including physicians, nurse practitioners, social workers, medical assistants, data analysts and other staff work together to provide seamless support to patients at every level of care.

The practice also offers patients on-site laboratory services, same-day appointments, extended hours and the ability to communicate securely with their physicians using an online patient portal.

This collaborative approach to primary care helps foster a more developed relationship between the healthcare team and the patient, and improves patient engagement. The end result: a better patient experience and improved clinical outcomes.

Optimizing workflows, improving clinical outcomes

The MHMG team re-engineered workflows so patients can see their physician within a time frame that best meets their needs:

  • 30 percent of each day’s appointments are reserved so that more patients get same day access and to accommodate “quick sick” appointments for patients currently in treatment 
  • Appointments are scheduled not more than three months in advance 
  • After Hours Clinic open evenings and weekends to improve access

All three tactics enable the MHMG staff to see patients when they need to be seen and address health issues before they become more serious.

The onsite lab makes it easier for patients to receive necessary screenings and timely treatment plan adjustments. As one example, proactive outreach has led to more than 50 percent of the practice’s patients with diabetes receiving screenings onsite to help them better manage their disease. 

MHMG uses technology and data to support the patient experience. Our patient portal connects patients with their healthcare team, test results and other personal health information.

Industry recognition

Placing the patient at the center of everything we do has earned My Health Medical Group recognition from the National Committee for Quality Assurance (NCQA) as a Level 3 patient-centered medical home. MHMG is the first practice in San Francisco to earn this status. It is the highest recognition possible from NCQA, a nonprofit healthcare accreditation entity that validates practices as patient-centered medical homes using a rigorous set of national industry standards.

In addition, My Health Medical Group was named the 2013 Patient-Centered Medical Home practice of the year by the California Association of Family Physicians.
