City and County of San Francisco

Accountable care initiative results in stabilizing premiums.

In just two years, Brown & Toland’s collaboration with Blue Shield of California and California Pacific Medical Center (CPMC) succeeded in reducing costs and improving the quality of care for 21,000 employees, dependents and retirees of the City and County of San Francisco. The positive impact contributed to no increase in health insurance premiums for the 2014 plan year — a result virtually unheard of in recent healthcare history.

Our arrangement serves City and County of San Francisco employees in the Blue Shield HMO who are assigned to a Brown &Toland physician and receive care at CPMC.

Employing an intense level of coordination, connectivity and care management, we are delivering a measurable impact:

  • 8.8 percent reduction in admissions per 1,000 members
  • 13.4 percent reduction in inpatient days per 1,000 members
  • 5.1 percent reduction in average length of stay for inpatient admissions (based on a comparison of year one of the ACO against the prior baseline year)

See how the power of collaboration fueled these tangible results.
