Comprehensive Diabetes Management
Managing diabetes on your own can be difficult. Keeping up with medications and blood glucose testing and watching your diet while carrying on with all of life’s demands can sometimes feel overwhelming. Free to our members, our diabetes experts can help you live well and successfully manage your condition.
Brown & Toland Physicians’ care management program offers highly coordinated in-home care that is designed to keep eligible members with complex health and social needs as healthy as possible. If you are eligible, our care management team will reach out to you and collaborate closely with your primary care physician (PCP).
The Palliative/Hospice program provides expert symptom management, ensuring patients’ wishes for their medical care are both known and honored, and helps patients and their caregivers cope with serious illness. Learn More

Registered nurses are available 24 hours a day, seven days a week to answer health questions for Brown & Toland Medicare Advantage and Medi-Cal patients.
