Medicare Pioneer Accountable Care Organization

Model program improves care quality and lowers costs

Brown & Toland Physicians was one of the initial 32 healthcare organizations selected to participate in the Pioneer Accountable Care Organization (ACO) model in late 2011. The transformative model is sponsored by the Centers for Medicare & Medicaid Services (CMS) Innovation Center.

Through our care coordination, connectivity and care management programs for this population, we have helped CMS save more than $17 million during our participation in the initiative.

Brown & Toland, the only healthcare organization in Northern California selected by CMS to participate, was chosen based on our proven ability to build comprehensive and clinically-integrated systems that connect healthcare providers, improve the quality of care provided and reduce costs. CMS provides incentives based on meeting quality standards such as patient outcomes and care coordination among healthcare provider teams.
